UAHA HOCKEY FEE/DUES ASSISTANCE PROGRAM 2016 – 2017 SEASON
The UAHA Board of Directors is pleased to announce that they are once again, for the fourth year in a row, offering a fee/dues assistance program to any eligible youth (18U) Utah hockey player in good standing for the 2016 – 2017 season. The assistance offered this year is up to $300 per player provided the Member Organization (Sub-Affiliate Team or Organization) matches the award from the Board of Directors for a total assistance of up to $600. The total of all awards cannot exceed $4200.
Funds shall be allocated to the Utah Hockey (UAHA) participant from the most financially disadvantaged family first, the next most financially disadvantaged family second, and so on until all available funds have been allocated. UAHA may also use certain other hardship cases, other than financial as it sees fit. If there are not sufficient applications to utilize the funds available the remaining funds shall be returned to the Utah Hockey General Fund.
The application deadline is September 30th 2016 at 5:00 pm. Allocations shall be made at the UAHA Board meeting on October 3rd, 2016. Application and questions must be emailed to Derrick Radke at firstname.lastname@example.org.
